NEW PATIENT INFORMATION
Urology Specialists of Coast Georgia, P.C. welcomes new patients. Our goal is to provide our patients with professional, up-to-date care and our doctor’s are knowledgeable in the field of Urology. We consider, “listening to the patient” as a major part of our practice.
Insurance companies consider the physician’s at Urology Specialists of Coastal GA, P.C., to be specialists. Thus, all new patients are required to have a referral from their primary care physician (PCP). We do not see Walk Ins or those who have not acquired a precert number from their insurance company (HMO’s, POS, etc.). Listed below is a detailed list of requirements before appointments can be made or continuity of care can be continued.
INSURANCE - Please check to see if we are covered by your insurance plan. If not, you may have to pay “out-of-network” charges, which can be rather expensive to you. There are two types of referral required: The first one is a form that your PCP fills out and then faxes to us with the necessary medical information. This is for patients with Medicare, PPO’s, self paying patients, etc. The second is the same form and medical records plus an insurance number that approves your visit to our office. This is for patients who have HMO’s, PHS, etc.
COPAYS - Insurance companies require copays to be paid at the time service is rendered. It is not our policy to bill for any copays.
MEDICAL RECORDS AND RADIOLOGY IMAGING - The referring physician will be required to send your medical records to us, before your scheduled appointment. All radiology reports and x-rays done pertaining to your medical condition are required for your scheduled visit. These can be obtained from the facility where the procedure was done. It is your responsibility to obtain these.
HOSPITAL AND LABORATORY - Your insurance company may require you to use a specific hospital or laboratory and you are required to know which one is appropriate for your plan. We cannot choose the hospital or laboratory for you, nor are we required to.
